I, as caregiver of my student, request that an Oak Tree Tutor be allowed to tutor/coach my student.
I understand that during the 2026-2027 academic year, tutoring sessions occurring in-person must be held at TRC's center. For virtual tutoring and coaching sessions, students and tutors agree to use the video-conferencing platform selected, which will be explained prior to the first tutoring session. Coaching check-ins and tutoring appointment reminders may occur by telephone, email, text, or other electronic means as mutually agreed by the tutor/coach and student.
I understand that my child may be recorded by building security cameras during in-person sessions. I also understand that virtual sessions may be transcribed. Oak Tree Tutors uses these recordings and transcripts solely to help ensure the safety and well-being of students and staff. All records are for internal use only.
In the event that my student or my student's tutor is unable to meet in-person due to illness, transportation, or other issues, an Oak Tree Tutors Staff Member may authorize sessions to be held virtually using TRC's authorized platform. I understand that I am responsible for the supervision of my student during any authorized virtual tutoring/coaching sessions.
I understand that when such services are rendered at Oak Tree Tutors, during these times the tutor/coach will have complete responsibility for the supervision of my student and will be working with my student in the programming space with Oak Tree Tutors supervision.
I understand that Oak Tree Tutors does not assume responsibility for the loss or damage of personal property, or any loss of whatever kind or nature incurred by persons using the facilities.
I acknowledge that students will abide by the values and posted norms of the organization during tutoring.
I understand that I am responsible for monitoring my student's progress.
I give permission for the tutor/coach to collect and share educational information and data with a designated Oak Tree Tutors staff, tutors, and coaches, and teacher(s) in the subject(s) listed above.
I understand that if the student has 3 "unexcused" absences, they may be dropped from the program to make room for others in need of tutoring.
I hereby release Oak Tree Tutors, its board of directors, staff, officers, contractors, and employees, and each of them, from any and all liability of whatever nature or kind arising from, or by reason of, any injury or damage which may befall me or my student while present on the Oak Tree Tutors premises, while making use of the organization's facilities, or receiving tutoring/coaching services virtually, including all risks connected therewith, whether foreseen or unforeseen, except where injury or damage results from the gross negligence of the school, its trustees, officers and employees.
